Slots are available for Saturdays and school holidays. If there is not availability to suit you please contact us and we will be happy to see if we are able to accommodate your requirements.

This assessment requires the completion of three questionnaires. Parents should fill out the parental questionnaire and also request that their child's school completes the school questionnaire. The school questionnaire will be emailed to you with confirmation of your appointment, once you have booked an assessment time. The links for the parental questionnaire are linked from this page.

Additionally, parents need to complete the visual difficulties protocol. An optometrist assessment may be necessary for some children to address any eyesight concerns prior to diagnosis. Filling out the visual difficulties protocol questionnaire before scheduling an appointment will help avoid delays in the assessment process.

The final diagnostic report can only be compiled after all questionnaires are fully completed and submitted.
